Apollo’s Multi-Disciplinary Medical Imaging Platform – arcc®

The Society for Imaging Informatics in Medicine (SIIM) annual conference took place in Austin, Texas, last month. Apollo had a booth in the exhibit hall, along with many other healthcare IT vendors. While looking for great BBQ in Austin was fun, what we found more interesting was most every vendor there referred to themselves as “Enterprise Imaging” (EI). What they meant by that term varied widely from vendor to vendor: The Radiology PACS vendors meant “ use our solution to provide an Enterprise PACS;” the Interface vendors meant, “we can tie all your systems together with our interface engine to create EI;” and other vendors really did not have a definition but had it as a headline topic on their booth display.

What does this mean, and how does Apollo stand out in a sea of companies promoting their definition of EI? As the week progressed, it became clear that we were the only healthcare IT vendor on the floor that could support whichever department participants were trying to solve problems for, or whatever imaging format was being addressed. Said another way: clinical workflows and tying all images and departments into a single patient record are vital to effective patient care.

The other thing that became apparent is that EI is now a ubiquitous term that everyone has adopted. As we have stated before, to be a true EI solution, you must address departmental workflows: how clinicians use our system to capture, view, store and make images available to other caregivers across the health system. The goal of an enterprise imaging strategy should be to improve your clinical workflows, provide multi-disciplinary access to all patient images, and enable you to gain efficiencies throughout your health system. As the Apollo Repository for Clinical Content, arcc, does just that.

Here at Apollo, we want to be sure customers and partners understand what EI solution we provide. To address that, we have thought about what we really are trying to provide for our customers, and what term is incredibly descriptive of Apollo arcc. The term is “Multi-Disciplinary Medical Imaging Platform“(M²IP). This term resonates immediately as it directly addresses the complexity that hospitals face with the amount of imaging of all kinds being generated across hospitals today. We won’t change the name arcc, but the description will be M²IP.

A few sneak peaks:

  • There’s more to patient imaging than just radiology and cardiology images such as xrays, MRIs, EKGs. There are many “visible light” images taken of patients that need to be stored, managed, accessed and shared as part of the patient care process.
  • Clinicians and hospital IT staff also have to “think outside the DICOM” with the workflow process – there is not always an order telling the clinician what images to take – POC image capture is important too.

what is enterprise imaging?

What is Enterprise Imaging? Why does it matter and what advantage does it bring to healthcare today?  The answer should be simple. Turns out it is not. Many imaging vendors are out there claiming to provide Enterprise Imaging because they have a PACS that can store and view Radiology (DICOM) images from other specialty PACS systems or have a VNA that can archive DICOM images. But, is that all there is to an Enterprise Imaging strategy?  The answer is a resounding NO! According to the  HIMSS-SIIM Enterprise Imaging Workgroup, the definition is:

“a set of strategies, initiatives and workflows implemented across a healthcare enterprise,” enterprise imaging is used “to consistently and optimally capture, index, manage, store, distribute, view, exchange, and analyze all clinical imaging and multimedia content to enhance the electronic health record.”

The gap between our competitors who are archiving images and Apollo comes down to a few simple things in the definition above:

The Workgroup recognized that Radiology isn’t the only department capturing and storing images and put language in to specifically address clinical multimedia. Radiology departments don’t want all the “other ology” images in their PACS, which was designed to aid a Radiologist to make a diagnosis. Radiologists rarely look at anything besides CT’s MRI’s, X-Rays, etc. They don’t want to be responsible for storing any other department’s images.

There are many vendors who offer a Universal Viewer, but, most of these are optimized for DICOM viewing, and even if they can view other image types, they often require secondary systems to view non-DICOM images such as JPEG, Tiff, and MP4.

PACS vendors are the best at capturing DICOM images in the Radiology department, and there is a whole ecosystem to make that happen. Where this fails is outside of Radiology. When you ask a wound care nurse to follow the Radiology workflow or ask an ER physician to create an order before doing a Point of Care Ultrasound, they won’t necessarily know how, have time, or have the access to do what you are asking them to do. Their workflow does not follow an “orders-based” workflow.  “Optimally” gets tossed out the window so someone in IT can archive an image.

The Apollo advantage is that we do everything the definition calls for! No secondary system requirements and no requirement to follow another department’s workflow!  Efficient systems drive effective care…the choice seems simple to us. What do you think? What is Enterprise Imaging? What do you think Enterprise Imaging means?

Apollo’s new podcast, “Think Outside the DICOM” will provide you with some Apollo thoughts on Enterprise Imaging related topics. 

Fine tuning the definition provided by the HIMSS-SIIM Enterprise Imaging Workgroup, there are a few key aspects that Apollo focuses on.

Enterprise Imaging is...“a set of strategies, initiatives and workflows implemented across a healthcare enterprise to consistently and optimally capture, index, manage, store, distribute, view, exchange, and analyze all clinical imaging and multimedia content to enhance the electronic health record.”

Electronic Medical Records (EMRs) are driving us to have more and more clinical information available and easier access to clinical information. So how does an enterprise imaging system do that?

  1. Can the system store and archive all medical images centrally, eliminating departmental silos?  Archiving is important but it’s not the only solution to Enterprise Imaging.
  2. Can the system support the workflows to make it easy to capture, save and view all types of clinical images?
  3. Can the system provide cooperation with the EMR as an integral part of the treatment workflow, image-enabling the EMR to ensure care collaboration across specialties and clinicians?

At Apollo, we think Enterprise Imaging comprises 3 components:

  1. Centrally managed storage access with images stored in native format
  2. Ability to connect the archive and images to the EMR
  3. Ability to drive clinical workflows – how are you capturing those clinical images? 

The solution should not just be an archive, it needs to support all specialties, including non-DICOM or visible light generating departments, across the enterprise, enable image acquisition workflows, store them in native format, and view them in native format. This is when you jump from being an VNA to a true clinical enterprise imaging system. 

The advantage of having an enterprise imaging strategy in place provides a more holistic, comprehensive view of patient imaging. There is a value in creating a timeline or story with clinical pictures. The universal use and access of the picture, and the story that can be told with that picture at a patient-centric level. It also suggests that tags or metadata can be added to these images easily, where in the past, it was not readily available. To have all of this accessible, when you are uniformly and centrally managing the information with an enterprise imaging solution, ensures care collaboration and creates efficient workflows that drive effective care
